How To: Document a CSE

Documenting is one of the first things I learned as a graduate student and is a very useful skill no matter what setting you are in. Here is an outline of how to document a clinical swallow examination AKA bedside swallow evaluation if you really have no clue where to start. Definitely take this template and make it your own - add, delete, change parts you like/don't like/or can improve on.
